IMPORTANT:
Applicants must enter accurate phone numbers and emails for themselves and thier references. Family members can be used as a reference.
After all required information is entered, a review page appears where the applicant can make edits into the application prior to the final submission. Once submitted the application is unableto be altered. When complete, the applicant must print out the application form, fingerprint form and confirmation page.
Payment shall be in the form of a Check or Money Order issued to 'Borough of Prospect Park' or credit card. NO CASH please.
