Initial Complaint Form
The City of Tangent Complaint Form must be filled out with all required fields completed. You may download the paper version to be printed, signed, and submitted to the City of Tangent in person or by e-mail. A complaint form without a signature will not be investigated.
Please note that the complaint will not be investigated if staff cannot read sloppy handwriting or contact the complainant.
